Payment Plan

As a registered patient you can sign up online to join our payment plan, at a time to suit you. When joining you will be eligible for the benefits of the plan when you have paid at least 4 monthly payment plan payments. If you have an appointment within the next 4 months you can instead pay the 4 monthly payments and the registration fee in the practice on the day of your appointment to have your check up, hygienist visit or emergency appointment covered.

You need to fill out 2 forms to join online. Once you have completed the form below you will be redirected to fill out the second direct debit form. Once both forms are completed we will write to you to confirm it has been set up.
